Description
You will have received a letter and an email with information needed to complete this Scholarship Award Acceptance Form. This is the form you must submit for your 2nd check payment. Your scholarship letter should reflect your payment schedule.
Upon submission of the form and required attachments, the information will be verified for completeness and accuracy. Upon approval, your request will be processed. Award check(s) will be sent via USPS first class mail and should be received in 10 - 14 days.
Once you have submitted this form, you won't be able to enter it again to make changes. If you need to accept a new/additional scholarship, you may create and submit a new form.
