Description

Every year, the Pharmacy Society of Wisconsin recognizes outstanding members of the Wisconsin pharmacy and healthcare community. PSW members may nominate a colleague for the awards below.

The Pharmacy Society of Wisconsin & Wisconsin Pharmacy Foundation welcome applications from those who represent diversity in race, color, ethnicity, culture, national origin, sexual orientation, age, gender, gender identity, social class, physical ability/attributes, religious or ethical value systems, language, pharmacy practice area, geographic location, and other perspective shaping backgrounds.

Requirements

All nominations must be fully completed and submitted to PSW by the nomination deadline in order for the nominee to be considered as a candidate for the award. To complete the nomination, please submit the following:
  • 3 Letters of Support
  • Resume/CV
Each qualified nomination will be evaluated based on the award's selection criteria. Documentation supporting a candidate's nomination should highlight how the candidate has fulfilled the award's selection criteria. All awards will be presented at the PSW Annual Meeting in the Fall of the nomination year. For questions or for more information, please contact the PSW office at (608) 827-9200 or info@pswi.org.
