2024 Pre-Qualifier for John W. Youtsey, Ph.D. Memorial Scholarship

Description
A Pre-Qualify form for the John W. Youtsey Memorial Scholarship.This form is only to determine your eligibility to apply for the John Youtsey Memorial Scholarship. The scholarship application period begins February 1, 2024. Once it is determined that you are qualified to apply, you will receive an email containing the link to the actual application. It is important to check your spam folder if you do not receive the email. If you do not receive the email in 72 hours it is your responsibility to contact Erica Hunter to inquire about the status of your Pre-Qualify form.
The actual scholarship application must be completely submitted by May 1, 2024 at 5PM.
Please make sure to fill out and submit the Pre-Qualify form ASAP to ensure HoG staff have enough time to send you the link for the John W. Youtsey, Ph.D. Memorial Scholarship application and you complete the application by May 1, 2024 at 5 PM.

Requirements
HoG Client Scholarship Eligibility Requirements:1. You must have Hemophilia, von Willebrand Disease, another inherited bleeding disorder, or be a surviving child of a parent who died as a result of complications arising from a bleeding disorder and who had been a client of HoG; and
2. You must currently reside in Georgia and have been a permanent resident of Georgia for a minimum of one year; and
3. You must be currently enrolled in an accredited college, university, vocational, technical, or professional degreed program, OR have been accepted by an accredited college, university, vocational, technical, or professional degreed program for the 2024-25 school year; and
4. If you are awarded a scholarship, you must agree to serve the bleeding disorder community or another deserving community by volunteering at least 12 hours by May 31, 2025. If you have volunteered at Camp Wannaklot, this will serve as the only volunteer work you need to complee to qualify for the scholarship.
5. You must have a qualifying GPA of at least 2.5.
